Medication Authorization Form
This form must be filled out and returned to the ACES site before your child's first day. The medication authorization form must be renewed each ACES session. Written parent permission and written physician orders are required before any medication is dispensed during the ACES program. Medications must be in their original container with proper label. Site Supervisors do not have access to the school nurse's office. Parents are responsible for transferring medication to the Non School Day/Summer Sites.
